摘要：目的 了解甘肃省静宁县新农合运行现状，发现存在的问题，提出相应对策和建议。方法 收集甘肃省静宁县2006-2015年间新农合年报表，分析该县新农合参合情况、资金平衡情况、次均费用、农民受益情况及参合农民住院费用负担情况。结果 农村居民参合率、筹资水平和受益水平逐年升高；县级以上医疗机构住院人次和基金补偿金额所占比例的快速增长是住院次均费用上涨的主要原因。结论 静宁县新农合制度运行总体良好。需进一步加强监管，建立分级诊疗和转诊制度，切实降低医疗费用和参合农民的医疗负担。

Analysis on the New Rural Cooperative Medical System from 2006 to 2011 of World Loan Health Ⅺ Project Pilot Jingning County of Gansu

Abstractive：OBJECTIVE To understand the county NCMS operation status, find the problems existing in the operation of new rural cooperative medical system, and puts forward the corresponding countermeasure and the suggestion. Methods The annual reports of NCMS in Jingning and relevant data were collected to dynamically analyze the participation rate, funding balance, fund raising and fund operation efficiency from 2006 to 2015. Results The Gansu province Jingning county new rural cooperative participation rate has been greatly improved; the use of capital tends to be reasonable; average expense increases year by year; benefit face gradually expanded; the burden of the farmer got mitigation. Conclusion The new rural cooperative medical system in Jingning county operates well. The supervision of the designated medical institutions should be further strengthened, the grading system of health care and two-way referral system should be set up, and the payment method reform should be carried out to effectively reduce health care cost and the burden of medical expense of farmers.

静宁县位于甘肃省中东部，是国家扶贫开发工作重点县，有24个乡镇,392个行政村, 总人口48.25万人，其中农业人口43.79万人。2006年推行新型农村合作医疗制度，2010年取消门诊家庭账户，实施门诊统筹。2008年，会宁县被卫生部列为世界银行贷款/英国政府赠款中国农村卫生发展项目（卫XI项目）试点县，在新农合门诊统筹、支付方式改革、基金管理和定点医疗机构监管等领域开展了创新试点。本研究利用甘肃省静宁县2006-2015年度新农合报表资料，通过对相关指标的进行分析，了解该县新农合运行状况和存在的问题，提出相应对策和建议，为进一步促进新农合制度改革发展提供科学依据。

1 材料与方法

1.1资料来源

（1）从甘肃省静宁县合作医疗管理局收集该县2006-2015年度《甘肃省新型农村合作医疗运行情况分析年报表》资料，进行相关指标的整理与分析。

（2）从2006-2015年甘肃省国民经济和社会发展统计公报数据中，获得2006-2015年静宁县国民经济和社会发展统计数据，包括基本人口和经济等相关指标。收集新型农村合作医疗制度相关政策文件，了解该县新农合制度相关背景及运行状况。

1.2统计方法（静宁县）

运用Excel2007建立数据库并进行整理，对静宁县2006-2015年新农合参合情况、筹资基金来源、筹资基金平衡、住院费用及筹资基金补偿受益等指标进行描述性统计分析。

2 结  果

2.1静宁县新农合参合情况

从2006年到2015年，静宁县农村居民参合率稳步提高，从2009年以后，一直保持在97%以上，高于同年度全国或甘肃省平均水平或持平^[1]（见表1）。
